Treatment-related gastrointestinal toxicities and advanced colorectal or pancreatic cancer:A critical update

摘要 Gastrointestinaltoxicities(GIT),includingoralmucositis,nauseaandvomiting,anddiarrhea,arecommonsideeffectsofchemotherapyandtargetedagentsinpatientswithadvancedcolorectalcancerandpancreaticcancer.Beingoftenunderreported,itisstilldifficulttopreciselyestablishtheirburdenintermsofbothpatient’squalityoflifeandcancercarecosts.Moreover,withtheuseofmoreintensiveupfrontcombinationregimens,thefrequencyofthesetoxicitiesisrapidlygrowingwithapotentialnegativeeffectalsoonpatient’soutcome,asaresultofdosereductions,delaysorevendiscontinuationofactivetreatments.Thus,identifyingpatientsathigherriskofdevelopingGITaswellasanoptimalmanagementareparamountinordertoimprovepatient’scomplianceandoutcome.Afterthedescriptionofthemaintreatment-inducedGIT,wediscussthecurrentknowledgeonthepathophysiologyofthesesideeffectsandcommentthescalescommonlyusedtoassessandgradethem.WethenprovideacriticalupdateonGITincidencebasedontheresultsofkeyrandomizedtrialsconductedinpatientswithmetastaticcolorectalcancerandadvancedpancreaticcancer.
