摘要
摘要目的探讨结核病性胸腔积液患者血清与积液中多项细胞因子水平的变化特征.方法临床纳入我院收治的胸腔积液患者７６例,其中结核性胸腔积液４８例,癌性胸腔积液２８例.对两组患者血清以及积液中的白介素－８(IL－８)、血管内皮生长因子(VEGF)、C反应蛋白(CRP)、肿瘤坏死因子－α(TNF－α)、干扰素－γ(INF－γ)水平的差异.结果结核组血清IL－８、VEGF、CRP、TNF－α、INF－γ水平分别为(３５．０±６．７)μg/L、(０．１±０．１)μg/L、(４２．１±１１．３)mg/L、(０．７±０．５)pg/L、(０．６±０．４)pg/L,恶性组分别为(５０．２±５．５)μg/L、(０．３±０．１)μg/L、(２４．３±１３．２)mg/L、(０．３±０．２)pg/L、(０．１±０．１)pg/L,差异均有显著性(P＜０．０５);结核组胸腔积液IL－８、VEGF、CRP、TNF－α、INF－γ水平分别为(９６．３±７．３)μg/L、(０．２±０．１)μg/L、(２５．３±９．５)mg/L、(１．９±１．７)pg/L、(１．１±０．５)pg/L,恶性组分别为(８０．２±７．７)μg/L、(２．７±０．８)μg/L、(１２．２±８．４)mg/L、(１．１±０．２)pg/L、(０．２±０．１)pg/L,差异均有显著性(P＜０．０５).结论IL－８、VEGF、CRP、TNF－α、INF－γ等细胞因子水平在诊断鉴别结核性胸腔积液与恶性胸腔积液中均有一定的应用价值,但是单用时敏感性及特异性均不理想,应进行联合检查.关键词结核;肿瘤;胸腔积液;细胞因子;特征VariationCharacteristicsofVariousCytokineLevelinSerumandPleuralofPatientswithTBPleuralEffusionAbstractObjectiveTodiscussthevariationcharacteristicsofvariouscytokinelevelinserumandpleuralofpatientswithTBpleuraleffusion．Method７６casesofpatientswithpleuraleffusioninourhospitalwereselected,４８caseswereTBpleuraleffusion,and２８caseswerecancerouspleuraleffusion．TheIL－８,VEGF,CRP,TNF－α,andINF－γlevelsofserumandpleuralofbothgroupswereobservedandcompared．ResultTheIL－８,VEGF,CRP,TNF－α,andINF－γoftheTBgroupwere(３５．０±６．７)μg/L,(０．１±０．１)μg/L,(４２．１±１１．３)mg/L,(０．７±０．５)pg/L,and(０．６±０．４)pg/L,while(５０．２±５．５)μg/L,(０．３±０．１)μg/L,(２４．３±１３．２)mg/L,(０．３±０．２)pg/L,and(０．１±０．１)pg/Lforthecancerousgroup．Thedifferencewassignificant(P＜０．０５);ThepleuralIL－８,VEGF,CRP,TNF－α,andINF－γoftheTBgroupwere(９６．３±７．３)μg/L(０．２±０．１)μg/L,((２５．３±９．５)mg/L,(１．９±１．７)pg/L,and(１．１±０．５)pg/L,while(８０．２±７．７)μg/L,(２．７±０．８)μg/L,((１２．２±８．４)mg/L(１．１±０．２)pg/L,and(０．２±０．１)pg/Lforthecancerousgroup．Thedifferencewassignificant(P＜０．０５);ConcluＧsionThecytokinelevelofIL－８,VEGF,CRP,TNF－α,andINF－γhavesomeapplicationvalueinthediagnosisofTBpleuraleffusionandcancerouspleuraleffusioKne．yHwoowrdesver,thesensibilityandspecificityofthesinglemethodisnotideal,whichmeanscombinationexaminationissuggested．TB,Tumor;Pleuraleffusion;Cytokines;Characteristics中图分类号R２４６．５文献标识码B文章编号１００８－６３１５(２０１５)１２－０５２１－０２
