摘要
摘要目的探讨持续封闭负压引流(vacuum－sealingdrainage,VSD)在骨科创伤中的临床疗效.方法选择我院２０１２年８月－２０１４年８月间收治的１００例复杂创伤性皮肤软组织缺损及开放性骨折术后感染患者,按照不同的治疗方法,将其随机分为试验组和对照组,对照组患者给予常规创面处理方式,试验组患者给予VSD技术进行治疗,对两组患者创口植皮创面愈合率、愈合时间及植皮次数进行分析,并比较两组患者的临床总有效率.结果试验组患者创口一次植皮创面愈合率为７２％,其余均在二次植皮中愈合,而对照组患者一次植皮创面愈合率仅为４４％,显著小于试验组(P＜０．０５),且对照组患者二次植皮中愈合１５例,另有１３例患者需要进行多次植皮;试验组患者创口愈合平均时间为(１５．２±５．４)天,明显大于对照组的(２３．４±７．２)天(P＜０．０５);试验组患者临床总有效率为１００％,也明显大于对照组的７８％(P＜０．０５).结论VSD技术治疗骨科创伤疗效确切,操作简便,可有效缩短皮肤缺损修复时间,同时减轻患者换药痛苦,值得临床的推广和应用.关键词骨科创伤;持续封闭负压引流;植皮;创面愈合率AbstractObjectiveToinvestigatethecontinuingVAC(vacuum－sealingdrainage,VSD)theclinicalefficacyoforthopedictrauma．MethodsinourhospitalinAugust２０１２－－２０１４inAugustadmitted１００casesoftraumaticcomplicatedskinandsofttissuedefectsandpostoperativeinfectioninpatientswithopenfractures,accordingtothedifferentmethodsoftreatment,theywererandomlydividedintoexperimentalandcontrolgroups,inthecontrolgroupweretreatedwithconventionalwoundtreatment,theexperimentalgroupweretreatedwithVSDtechnologyfortreatment,patientswereskingraftwoundhealingrate,healingtimeandthenumberofskingraftingwereanalyzedandcomparedtwogroupsofpatientsoverallclinicaleffectiveness．Resultsinpatientswithwoundhealinggraftoncethetestgroupwas７２％,therestwereinthesecondaryskingrafttoheal,whilethecontrolgroupofpatientswithaskingraftwoundhealingratewas４４percent,significantlylessthantheexperimentalgroup(P＜０．０５),andthecontrolgroupSecondaryskingraftinginpatientswithhealedin１５cases,while１３patientsrequiredmultipleskingrafting;woundhealinginthetestgroupofpatientsthemeantimewas(１５．２±５．４)days,significantlyhigherthanthecontrolgroup(２３．４±７．２)days(P＜０．０５);clinicaltrialinpatientswithtotaleffectiveratewas１００％,significantlyhigherthan７８％inthecontrolgroup(P＜０．０５)．ConclusionVSDtreatmentoforthopedictraumaefＧfective,easytooperate,caneffectivelyreducetheskindefectrepairtimeandreducepatientpainmedication,worthyofpromotionandclinicalapplications．Keywordsorthopedictrauma;continuousclosedsuctiondrainage;grafting;healingrate中图分类号R６８３文献标识码B文章编号１００８－６３１５(２０１５)１２－０８０１－０２
