摘要
摘要目的观察双侧股神经阻滞麻醉对老年双膝关节置换术患者血流动力学和苏醒时间的影响.方法选择拟进行双膝关节置换术的老年患者共４０例,随机分为观察组和对照组,每组２０例.对观察组患者实施双侧股神经阻滞麻醉,对对照组患者实施全身麻醉.记录两组患者相关时间点的HR及MAP(平均动脉压),包括T１(麻醉前)、T２(全麻诱导前)、T３(全麻诱导)、T４(手术开始)、T５(手术开始６０min)、T６(手术开始１２０min)、T７(患者苏醒时),观察患者的心血管事件等的发生.观察患者的苏醒时间.结果两组患者均实现安全麻醉及手术.两组的HR及MAP在麻醉诱导之后均降低,手术的过程中均基本保持平稳,控制于正常范围内,两组进行同时点比较的结果无统计学差异,即P＞０．０５.观察组苏醒时间比对照组短,两组比较的差异有统计学意义(P＜０．０５).观察组患者术中有１０例应用了阿托品或者麻黄碱,３例应用降压药或者降心率药,手术结束当天出现嗜睡者１例,对照组上述三项数据分别是１６、７、６例,两组比较有统计学差异,P＜０．０５.结论双侧股神经阻滞麻醉对老年双膝关节置换术效果好,可以稳定患者血流动力学,缩短患者的术后苏醒时间.关键词股神经阻滞麻醉;老年;双膝关节置换术;血流动力学;苏醒时间AbstractObjectiveToobservetheeffectofbilateralfemoralnerveblockanesthesiaonhemodynamicsandrecoverytimeinelderlypatientswithdoublekneereplacementsurgery．Methodsatotalof４０elderlypatientswithkneereplacementsurgerywererandomlydividedintoobservationgroupandcontrolgroup,２０casesineachgroup．Thepatientsintheobservationgroupweretreatedwithbilateralfemoralnerveblockanesthesia,andthecontrolgroupweretreatedwithgeneralanesthesia．TheHRandMAP(meanarterialpressure)oftwogroupswererecorded,includingT１(preanesthesia),T２(generalanesthesia),T４(T３),T５(６０min),T６(１２０min),T７(SuXingshi),andobservedtheoccurrenceofcardiovascularevents．Observethepatient＇srecoverytime．Resultstwogroupsofpatientswereachievedsafeanesthesiaandsurgery．TwogroupsofHRandMAPweredecreasedafteranesthesiainduction,andtheprocessoftheoperationreＧmainedstable,controlinthenormalrange,thetwogroupswerecomparedwiththeresultsofnosignificantdifference,thatis,P＞０．０５．Therecoverytimeoftheobservationgroupwasshorterthanthatofthecontrolgroup,andthedifferencebetweenthetwogroupswasstatisticallysignificant(P＜０．０５)．Observationof１０patientsweretreatedwithatropineorephedrinegrouppatients,３casesofapplicationofantihypertensivedrugsordropratemedicine,surgeryattheendofthedaysleepinessin１cases,controlgroup,theabovethreedataare１６,７,６cases,twogroupscomparedwithstatisticaldifference(P＜０．０５)．ConclusionbiＧlateralfemoralnerveblockanesthesiaintheelderlypatientswithdoublekneereplacementsurgery,canstabilizethehemodynamics,shortenthepatient＇srecoverytime．Keywordsfemoralnerveblockanesthesia;elderly;doublekneearthroplasty;hemodynamics;recoverytime中图分类号R３文献标识码B文章编号１００８－６３１５(２０１５)１２－０５２３－０２
