摘要
摘要目的探讨右美托咪定复合局麻用于侧脑室外引流术的临床效果.方法１０８例需脑室钻孔引流手术而不能配合局麻和手术的患者随机分为A、B、C三组,分别辅以右美托咪定、咪达唑仑、丙泊酚,观察三组用药前、用药后、术中最高、最低SBP、DBP、HR、SpO２,术中体动、呼吸暂停、是否辅助呼吸;评价术后较麻醉前意识状态加深率.结果A、B、C三组麻醉手术依从性、手术麻醉医师满意度无特殊差异(P＞０．０５);A组与B、C组低血压的发生率无明显差异(P＞０．０５);术后较麻醉前意识状态加深率B组大于A、C组(P＜０．０５);B、C组发生呼吸抑制、需辅助呼吸几率大于A组;A组出现心动过缓几率大于B、C组.结论右美托咪定复合局部麻醉能安全有效并更好地满足神经外科侧脑室外引流等短小手术的麻醉需求.关键词右美托咪定咪达唑仑丙泊酚侧脑室外引流AbstractObjectiveToinvestigatetheeffectsofdexmedetomidinecombinedwithlocalanesthesiafortheclinicaleffectofdrainageoflateralventricle．Methods１０８casesofventriculardrainageoperationneednotwithlocalanesthesiaandoperationwererandomlydividedintoA,B,Cthreegroups,respectively,withdexmedetomidinemidazolam,propofol,observethreegroupsbeforetreatment,aftertreatment,thehighest,lowestSBPDBP,HR,SpO２,bodymovement,apnea,whetherassistedrespirationduringoperation;evaluationafteroperationthanbeforeanesthesiastateofconsciousnessdeepenrate．ResultsA,B,Cthreegroupsofanesthesiaoperationcompliance,operationanesthesiologistssatisfactionnospecialdifference(P＞０．０５);AgroupandB,Cgroup,theincidenceofhypotensionhadnosignificantdifference(P＞０．０５);afteroperationthanbeforeanesthesiastateofconＧsciousnessdeepenrateingroupBthaningroupC(A,P＜０．０５);B,Cgroupofrespiratoryinhibition,assistedrespirationratehigherthanthatofgroupA;groupAslowheartbeatrategreaterthanB,Cgroup．ConclusionsDexmedetomidinecombinedwithlocalanesthesiaissafeandeffectiveandbetterkemyewetortdheneedsofanesthesiaforventricledrainageoperationinDepartmentofNeurosurgery．dexmedetomidinemidazolampropofolventricledrainage中图分类号R７１３文献标识码B文章编号１００１－５３０２(２０１５)０９－０７００－０１
