摘要
摘要目的对比远端胃癌行腹腔镜辅助D２根治术与传统开腹术的近期疗效.方法选取我院进展期胃癌并行远端胃癌根治术患者共１０５例,分别采用不同疗法,比较两组患者近期临床效果.结果腔镜组患者的各项临床手术指标数据均优于开腹组患者(P＜０．０５);获检淋巴结总数、肿瘤距远近切缘的距离与开腹组差异无统计学意义(P＞０．０５).腹腔镜组术后短期并发症发生率更低,但无统计学差异.结论腹腔镜辅助远端胃癌D２根治术效果显著,安全性好,且具有微创优势.关键词进展期胃癌;腹腔镜;传统开腹术AcomparativestudyonlaparoscopicBassistedandopendistalgastrectomyforadvancedgastriccancer．YANGBai－ren,LULing－jun,HUANGJun(DepartmentofGeneralSurgery,theFirstPeople??sHospitalofYibin,Sichuanprovince,China)AbstractobjectiveTocomparetheshort－termefficacyoflaparoscopicassistedD２radicalresectionandtraditionalopensurgeryinthetreatmentofdistalgastriccancer．Methodsatotalof１０５patientswithadvancedgastriccancerinourhospitalwereselected,andtwogroupsofpatientswithdifferenttreatmentwerecompared．Resultslaparoscopicgroupofpatientswiththeclinicalindicationsforsurgerydatawerebetterthanlaparotomygrouppatients(P＜０．０５);wontheexＧaminationoflymphnodecount,tumordistanceofcuttingedgedistanceandopengroup,thedifferencehadnostatisticalsignificance(P＞０．０５)．Theshort－termpostoperativecomplicationsoflaparoscopicgroupwerelower,buttherewasnosignificantdifference．ConclusionlaparoscopicassisteddistalgastriccancerradicalresectiKoenyofwogradsstriccancerhasasignificanteffect,goodsafety,andhastheadvantagesofminimallyinvasiveD２．advancedgastriccancer;laparoscopicsurgery;Conventionalopengastrectomy中图分类号R３２２．４＋４文献标识码A文章编号１００８－６３１５(２０１５)１２－０７７７－０２
