3.0T磁共振弥散加权成像在前列腺癌诊断中的临床应用

摘要   【摘要】 目的:探讨分析 3.0T磁共振弥散加权成像 DWI对前列腺常见疾病的诊断与鉴别诊断的应用价值。方法: 选取 76例前列腺疾病患者,其中 BPH患者 43例,前列腺癌患者 33例。所有患者均进行磁共振常规扫描和 DWI扫描,分析前列腺增生( BPH)和前列腺癌的 DWI和表观扩散系数 ADC图表现,并测量癌肿区的 ADC值及 BPH的平均 ADC值,进行统计学分析。 结果: BPH的 DWI和 ADC图上外周带信号均匀,中央腺体区信号不均匀;前列腺癌病灶在 DWI上呈高信号, ADC图呈低信号,受累的精囊,淋巴结转移和骨转移病灶 DWI上呈高信号, ADC图上呈低信号;癌区 ADC值约为( 0.62士 0.18) × 10-?mm2/s,增生区 ADC不直约( 1.43士 0.14) × 10-?mm2/s,两者差异有统计学意义( P < 0.05)。结论: DWI检查在前列腺相关疾病诊断中有特征性表现,并对前列腺癌和前列腺增生具有鉴别诊断价值。    【关键词】 3.0T磁共振弥散加权成像;前列腺癌;应用价值  [Abstract] Objective: To investigate the value of DWI in the diagnosis and differential diagnosis of common prostate diseases. Methods: 76 patients with prostate disease were selected, including 43 BPH patients and 33 prostate cancer patients. All patients underwent conventional MRI and DWI scan to analyze the DWI and apparent diffusion coefficient ADC of BPH and prostate cancer, and to measure the ADC value of cancer area and the average ADC value of BPH for statistical analysis. Results: on DWI and ADC of BPH, the signal of peripheral band was uniform, and that of central gland area was not uniform; on DWI, the signal of prostate cancer was high, on ADC, the signal of involved seminal vesicle, lymph node metastasis and bone metastasis was low, on DWI, on ADC, the signal of cancer area was (0.62 ± 0.18) × 10 -? Mm2 / s, and that of hyperplasia area was (1.43 ± 0.14) × 10 -? Mm2 / s The difference was statistically significant (P < 0.05). Conclusion: DWI has the characteristics in the diagnosis of prostate related diseases, and has the value of differential diagnosis for prostate cancer and hyperplasia.
