简介:AIM:Tocombinecommunityandhospitalservicesinordertoenableimprovementsinpatientmanagement,anintegratedgastroenterologyservice(IGS)wasestablished.METHODS:Referralpatternstospecialistclinicswereoptimized;openaccessrouteforendoscopicprocedures(includingesophago-gastro-duodenoscopy,sigmoidoscopyandcolonoscopy)wasestablished;familyphysicians’knowledgeandconfidencewereenhanced;directcommunicationlinesbetweenexpertsandprimarycarephysicianswereopened.Continuingeducation,guidelinesandagreedinstructionsforreferralwerepromotedbytheIGS.SixqualityindicatorsweredevelopedbytheDelphimethod,rigorouslydesignedandregularlymonitored.Improvementwasassessedbycomparing2010,2011and2012indicators.RESULTS:Anintegrateddeliverysysteminaspecificmedicalfieldmayprovideasolutiontoafragmentedhealthcaresystemimpairedbyalackofcoordination.InthispaperwedescribeanewintegratedgastroenterologyserviceestablishedinApril2010.Waitingtimeforproceduresdecreased:3moinApril30th2010to3wkinApril30th2011andstayedbetween1-3wktillDecember30th2012.Averagecostforpatient’svisitdecreasedfrom691to638NIS(adecreaseof7.6%).Sixhealthindicatorswereimprovedsignificantlycomparing2010to2012,2.5%to67.5%:Bonedensitometryforpatientswithinflammatoryboweldisease,preventivemedicationsforhighriskpatientsonaspirin/NSAIDs,colonoscopyfollowingpositivefecaloccultbloodtest,gastroscopyinBarrett’sesophagus,documentationoffamilyhistoryofcolorectalcancer,andcolonoscopyinpatientswithafamilyhistoryofcolorectalcancer.CONCLUSION:EstablishmentofanIGSwasfoundtoeffectivelyimprovequalityofcare,whilebeingcosteffective.

简介:Titaniumnitride(TiN)asarefractoryplasmonicmaterialisproposedtobeusedasanangle-insensitiveintegratedbroadbandsolarabsorberandnarrowbandnear-infrared(NIR)emitterforsolarthermo-photovoltaic(STPV)application.ByconstructingTiN-nanopatterns/dielectric/TiNstackmetamaterial,approximately93%lightabsorptioninawavelengthrangeof0.3–0.9μmandnearunitnarrowband(Δλ∕λ~0.3)emissioninNIR(~2μm)weredemonstratedbynumericalsimulation.Keepingtheexcellentlightabsorptioninthevisibleband,theemissionwavelengthcanbeeasilytunedbypatterningthetopTiNlayerintovarioussubwavelengthstructures.ThisdualfunctionattributestotheintrinsicabsorptionandplasmonicpropertyofTiN.Insuchanintegratedstructure,broadbandabsorptionandnarrowbandemissionneedtobebalancedforanoptimizedpowerefficiencyconversion.DetailedanalysishasdemonstratedthattheSTPVsystembasedonthisintegratedabsorber/emittercanexceedtheShockley–Queisserlimitat1000K.
简介:Background:Bioenergyisre-shapingopportunitiesandimperativesofforestmanagement.Thisstudydemonstrates,throughacasestudyinScotspine(PinussylvestrisL.),howforestbioenergypoliciesaffectstandmanagementstrategies.Methods:Optimizationstudieswereexaminedfor15Scotspinestandsofdifferentinitialstanddensities,sitetypes,andtemperaturesumregionsinFinland.StanddevelopmentwasmodeledusingthePipeQualstandsimulatorcoupledwiththesimulation-optimizationtoolOptiForBioenergytoassessthreeforestbioenergypoliciesonenergywoodharvestfromearlythinnings.Results:Theoptimalsolutionsmaximizingbarelandvalueindicatethatconventionalforestmanagementregimesremainoptimalforsparsestands.Energyharvestsoccurredonlywhenprofitable,ledtolowerfinancialreturns.Aforestbioenergypolicywhichincludedcompulsoryenergywoodharvestingwasoptimalfordenserstands.Atahigherinterestrate(4%),increasingenergywoodpricepostponedenergywoodharvesting.Inaddition,ourresultsshowthatearlythinningsomewhatreducedwoodqualityforstandsinfertilesites.Forlessfertilesites,thechangeswereinsignificant.Conclusions:Aconstraintofprofitableenergywoodharvestisnotrational.Itisoptimaltocarryoutthefirstthinningwithaflexibleforestbioenergypolicydependingonstanddensity.

简介:Weproposeanultra-simpledual-channelconfigurationforsimultaneouslyevaluatingtwobranchesofamultifunctionalintegratedopticchip(MFIOC).Intheconfiguration,theMFIOCisemployedasabeamsplittertoconstructthedemodulationinterferometertogetherwitha2×2fibercoupler.InterferencehappensbetweenpolarizationmodestravelingthroughdifferentchannelsoftheMFIOC.Thecross-couplingsofeachchannelarerespectivelycharacterizedbytheinterferencepeakswhichdistributeonoppositesidesofthecentralinterferencepeak.TemperatureresponsesoftheMFIOCareexperimentallymeasuredfrom-40°Cto80°C.Resultsshowthattheproposedconfigurationcanachievesimultaneousdual-channeltransientmeasurementswithresolutionof-90dBanddynamicrangeof90dB.Inaddition,thetwochannelsoftheconfigurationhaveconsistentmeasuringperformance,andthetwobranchesoftheMFIOChavedifferentresponsestotemperaturevariation.
