简介：摘要目的总结白内障超声乳化术中发生的灌注液迷流综合征的临床表现和治疗方法.方法２０１１－０１/２０１５－０４眼一科白内障超声乳化手术患者有４例发生灌注液迷流综合征.这些患者的临床表现为突发性眼压升高、浅前房、虹膜脱出,通过注入黏弹剂以及升高灌注瓶高度无法维持前房深度.所有患者未发生暴发性脉络膜出血.１例患者术中未予特殊处理继续进行手术操作,３例患者经过缝合主切口,包眼安静休息联合２０％甘露醇注射液快速静脉滴注处理.结果未予特殊处理的１例患者手术中发生后囊膜破裂,进行前段玻璃体切除,睫状沟置入人工晶状体.经过处理的３例患者中有１例经过处理后前房仍无法形成,故经睫状体平坦部玻璃体腔穿刺;其余患者前房形成.所有患者在继续进行的手术中均尽量减少前房灌注液流量、流速,手术完成后缝合主切口一针.结论超声乳化术中灌注液迷流综合征是一种少见的白内障术中并发症,经过恰当处理,可以顺利解决高眼压状态,完成手术.关键词灌注液迷流综合征;白内障超声乳化;表现;治疗方法AbstractAIMTosummarizetheclinicalmanifestationandtreatmentofinfusionmisdirectionsyndromeduringphacoemulisification．METHODSInfusionmisdirectionsyndromeoccurredin４casesundergoingcataractsurgeryinourhospitalfromJanuary２０１１toApril２０１５．TheclinicalmanifestationsofthesepaＧtientsincludesuddenelevationofintraocularpressure,shallowanteriorchamber,irisprolapse,RESULTSTheonecase,withoutspecialtreatment,wasperＧformedanteriorvitrectomyandintraocularlenssuspensionbecauseofposteriorcapsularruptureduringoperation．Aftertreatmentofother３patients,１patientwasstillunabletoformtheanteriorchambers,soparsplanavitreouscavitypuncturewascarriedout;othersformedanteriorchamber．AllthepatientsweresuＧturedmainincisionaftersurgery．CONCLUSIONSInfusionmisdirectionsyndromeisararecomplicationduringphacoemulisification,butthesurgerycanbecomＧpletedKseuyccweosrsfdusllyafterintraocularpressureproperlyhandled．Infusionmisdirectionsyndrome;Phacoemulisification;Clinicalmanifestationl;Treatment中图分类号R７７６文献标识码B文章编号１００８－６３１５(２０１５)１２－０５３５－０１
