简介：AIM:Toevaluatetheefficacy,safetyandstabilityofposteriorchamberphakicintraocularlensimplantationforthecorrectionofhighmyopia.METHODS:Retrospectivecasereviewof82eyes(43patients)undergoingimplantableColiamerlens(ICL)placementbyasinglesurgeon(Xiao-WeiGao)tocorrectpreoperativemeansphericalequivalentsbetween-9.00diopter(D)and-23.00D.Mainoutcomemeasuresincludeduncorrectedvisualacuity(UCVA),refraction,bestspectacle-correctedvisualacuity(BSCVA),endothelialcelldensity(ECD),intraocularpressure(IOP),lenstransparency,postoperativeuveitis.Visanteanteriorsegmentopticalcoherencetomography(AS-OCT)wasusedtomeasureanteriorchamberdepth(ACD)andthepositionofICL.RESULTS:Meanfollow-upwas6.54±3.26months(range3-12months).Predictabilityofthemanifestsphericalequivalent(SE)refractiontowithin±1.OODwasachievedin88%ofeyesand±0.50Din72.5%ofeyes.ThemeanpostoperativemanifestSErefractionwas-1.85±0.72D,with96.34%ofeyesmaintainingorgaining≥1line(s)ofBSCVA.Themean3-monthpostoperativeECDdecreasedbuthadnostatisticallydifferencecomparedwiththepreoperativeECD.Ofthe7eyes(8.54%)withamildtransientincreaseinintraocularpressure(upto30mmHg),nonerequiredasecondsurgicalprocedureorprolongedtopicalmedication.Therewasnolossoflenstransparency.Pigmentedprecipitateswereobservedin5eyes(6.09%).ThemeanpreoperativeACDmeasuredwithAS-OCTwas3.28±0.14mm,threemonthsaftersurgery,themeanACDwas2.45±0.22mm.Anteriorchamberdepthshowedastatisticallysignificantreduction.Oneeye(1.22%)hadICLspontaneousrotation,81eyes(98.78%)ofthelensremainedcorrectlycentered.CONCLUSION:TheimplantationofICLisaneffectivesurgicaloptionforthemanagementofhighmyopia.Butitslongtimeeffectandsafetystillneedmoretimetoprove.

简介：AIM:Toassesstheeffectivenessofimmunosuppressantsintheprophylaxisofcornealallograftrejectionafterhigh-riskkeratoplastyandnormal-riskkeratoplasty.METHODS:WesearchedtheCochraneCentralRegisterofControlledTrials(CENTRAL),MEDLINE,EMBASE,CNKI,VIPandreferencelistsofarticles.Dateofmostrecentsearch:18June,2011.Allrandomisedcontrolledtrials(RCTs)assessingtheuseofimmunosupressantsinthepreventionofgraftrejection,irrespectiveofpublicationlanguage.Twoauthorsassessedtrialqualityandextracteddataindependently.Onlydichotomousoutcomes(cleargraftsurvival,ratioofimmunereactionsandsideeffects)wereavailableandwereexpressedasrelativerisk(RR)and95%confidenceintervals(CI).RESULTS:Sevenstudieswereincludedinthisreview.Inthecomparingofmycophenolatemofetil(MMF)withplacebo,theresultsshowedMMFcouldsignificantlyreduceimmunereactionscomparedwithplacebo(RR1.0895%Cl0.95to1.21),butnoeffectoncleargraftsurvival(RR1.1195%Cl0.90to1.35).Incleargraftsurvivalandimmunereactions,MMFandcyclosporineA(CsA)showedsimilareffect(RR1.1195%Cl0.90to1.35,andRR1.48,95%Cl0.56to3.93,respectively).Tacrolimus(FK506)andsteroidshowedsimilareffectsoncleargraftsurvivalandimmunereactions(RR0.32,95%CI0.02to6.21,andRR1.00,95%CI0.88to1.14,respectively).Nodrugrelativesideeffecthasbeenfound.CONCLUSION:MMFmayreduceimmunereactionsinbothnormal-riskandhigh-riskrejectionofpenetratingkeratoplasty.CsAandFK506showedsimilareffectsasMMF.However,duetothelackoflargeclinicaltrials,theevidenceremainweak,thequalityofevidenceswereratedasverylowtomoderate.Large,properlyrandomised,placebo-controlled,doublemaskedtrialsareneededtoevaluatetheeffectofimmunosuppressants.

简介：目的:用过碘酸雪夫氏(PeriodicAcid-Schiff,PAS)染色法比较4%多聚甲醛固定液、4%戊二醛固定液和Davidson固定液固定豚鼠眼球的固定效果,筛选最佳眼球固定液和固定时间。方法:取正常豚鼠眼球分6组,每组5只,I组眼球放入4%多聚甲醛固定液固定24h、域组眼球放入4%戊二醛固定液固定24h;Ⅲ~V组眼球放入Davidson固定液分别固定3、6、24h;遇组眼球在Davidson固定液中固定3h后转移到10%中性甲醛中再固定48h。常规制片、PAS染色、显微镜观察,比较不同固定方法对组织的固定效果。结果:Davidson固定液固定3h的固定效果最为理想,Davidson固定液固定3h后转移到10%中性甲醛再固定48h的固定效果与Davidson固定液固定3h的固定效果接近,这两组固定液固定的眼球切片均结构完整、层次清晰,视网膜各层细胞排列整齐。结论:Davidson固定液对豚鼠眼球的固定效果明显优于其他两种固定液,豚鼠眼球用Davidson固定液固定后,可将其转移到中性甲醛中长期保存,其固定效果不受影响。

简介：AIM:ToevaluatetheeffectofCollagencross-linkingonthepreventionofmeltinginrabbitcorneasafteralkaliburn.·METHODS:TwentyNewZealandwhiterabbitswererandomlydividedintomodelcontrolgroupandcollagencross-linkingtreatmentgroup.Thesecondgroupofrabbitsreceivedcollagencrosslinkedtreatment.Bothgroupswereappliedwithantibioticeyedropstopreventinfection.Thecorneaswereevaluatedformelting,opacity,pathologicalandimmunohistochemistry,recordthechangeswhen28daysaftertheanimalswerekilled.·RESULTS:Inthecontrolgroup,6outof8rabbitsshowedcornealmeltingafterinjury(14±4)days,whiletwocornealperforated.Incollagencross-linkingtreatmentgroup,onerabbitshowedcornealmeltingafterinjury23days,withoutcornealperforation;cornealdissolutionratebetweenthetwogroupswassignificantlydifferent(P<0.05).Pathologicalexaminationsuggestedthatinthetreatmentgroup,mildcornealedema,milddamagetocollagenfibers,inflammatorycellinfiltrationwassignificantlylessthanthecontrolgroup.Immunohistochemistryshowedthatcornealcollagenfibersarrangedinneatrowsinthecontrolgroup.·CONCLUSION:Collagencross-linkingtreatmentnotonlycanpreventanddelaythecornealmeltingafteralkaliburn,butalsocanreducethedestructionofcornealcollagenfibersandinfiltrationofinflammatorycellsinthecornealtissue.

简介：目的观察自制全结膜囊羊膜固定器在重症眼表烧伤行羊膜遮盖手术中应用效果。方法在11例（13眼）眼部重症烫伤患者治疗中采用自制全结膜囊羊膜固定器将羊膜固定于眼表及眼睑皮肤面。根据病情需要更换羊膜，随访3月至3年。结果患者平均手术时间为（8．0±3．0）分钟；眼表组织上皮化时间（13．2±11．3）天。重复手术次数3．5次，患者手术依从性良好，观察病例无角膜穿孔，无睑球粘连。结膜囊狭窄5例，角膜斑翳5例，角膜血管化2例。结论采用自制全结膜囊羊膜固定器手术，缩短手术时间，无创伤，重症烧伤患者可多次重复手术，特别适合重症眼表烧伤早期治疗，减少晚期严重并发症的发生。
