  • 简介:Cardiaccatheterizationandpercutaneouscoronaryinterventionplayanimportantroleinthemanagementofcoronaryarterydisease.Althoughthetransfemoralapproachhasbeenthetraditionallydominantmethod,therehasbeenanincreasedutilizationofthetransradialapproach.Multipleobservationalstudiesandrandomizedclinicaltrialshaveshownfewerbleedingcomplications,reducedmorbidityandmortality,improvedqualityoflife,andbettereconomicoutcomeswhenthetransradialapproachisutilizedwhencomparedtothetransfemoralapproach.Despiteitsmanybenefits,utilizationofthisapproachincertaincountriesincludingtheUnitedStateshasbeenlessthanoptimalduetoaloweradoptionratesmostlydrivenbylackoftrainingopportunitiesanddecreasedawarenessofclinicalbenefitsofthetransradialapproach.Inthisreview,thehistory,observationaltrends,efficacy,andtechnicalaspectsoftransradialcardiaccatheterizationandpercutaneouscoronaryinterventionarediscussed.

  • 简介:Percutaneouscoronaryintervention(PCI)improvessymptomsandprognosisinischemia-inducing,functionallysignificant,coronarylesions.Useoffractionalflowreserveallowsphysicianstoinvestigatetheischemia-inducingpotentialofaspecificlesionandcanbeusedtoguidecoronaryrevascularization,especiallyinmultivesselcoronaryarterydisease.Fractionalflowreserve-guidedPCIhasbeenextensivelyinvestigated.Resultsshowthatdeferralofstentinginnon-significantlesionsissafe,whereasdeferralofstentinginfunctionallysignificantlesionsworsensoutcome.FFR-guidedPCIimprovesoutcomeinmultivesseldiseaseoverangiography-guidedPCI.Untilrecently,therewaslittleknownaboutthelong-termoutcomeofFFR-guidedrevascularizationanditsvalidityinacutecoronarysyndromes.Thisreviewaimstoaddressthenewevidenceregardinglong-termappropriatenessofFFR-guidedPCI,theneedforhyperemiatoevaluatefunctionalseverity,andtheuseofFFRinacutecoronarysyndromes.

  • 简介:Theadipocytokine,apelin-13,isanabundantlyexpressedpeptideinthenervoussystem.Apelin-13protectsthebrainagainstischemia/reperfusioninjuryandattenuatestraumaticbraininjurybysuppressingautophagy.However,secondaryapelin-13effectsontraumaticbraininjury-inducedneuralcelldeathandblood-brainbarrierintegrityarestillnotclear.Here,wefoundthatapelin-13significantlydecreasescerebralwatercontent,mitigatesblood-brainbarrierdestruction,reducesaquaporin-4expression,diminishescaspase-3andBaxexpressioninthecerebralcortexandhippocampus,andreducesapoptosis.Theseresultsshowthatapelin-13attenuatessecondaryinjuryaftertraumaticbraininjuryandexertsaneuroprotectiveeffect.

  • 简介:BackgroundMonocytetohighdensitylipoproteinratio(MHR)hasbeenconsideredasanovelparameterrelatedwithadverserenalandcardiovascularoutcomes.InthisstudyweinvestigatedtheassociationofMHRwithmajoradverseclinicalevents(MACEs)inpatientswithtype2diabetesmellitus(T2DM)undergoingelectivepercutaneouscoronaryintervention(PCI).MethodsConsecutiveT2DMpatientstreatedwithelectivePCIwereprospectivelyrecruitedbetweenJuly2008-January2016inDepartmentofCardiologyofPanyuCentralHospital.Subjectswerecategorizedintotwogroups:aspatientswhodevelopedMACEs(MACEs+)andpatientswhodidnotdevelopMACEs(MACEs-)duringhospitalization.MACEsweredefinedasthecompositeendpoints,includingall-causemortality,oracuteheartfailure,ortargetvesselrevascularization,orstrokeorrecurrentangina.ResultsAtotalof418patientswereincludedinthestudy.64patientsdevelopedMACEs(15.3%).IntheMACEs(+)patients,monocyteswerehigher(1.12[0.78-1.42]vs.0.72[0.68-0.92]109/L,P<0.01)andHDLcholesterollevelswerelower(0.87[0.72-1.21]vs.0.96[0.81-1.11]mmol/L,P<0.01).Inaddition,MHRwassignificantlyhigherintheMACEs(+)group(1.12[0.91-2.09]vs.0.73[0.54-0.93]109mmol/L,P<0.01).ThecutoffvalueofMHRforpredictingMACEswas22,withasensitivityof81%andaspecificityof75.1%(areaunderthecurve0.79,P<0.001).Inmultivariatelogisticregressionanalysis,MHRremainedanindependentfactorcorrelatedwithMACEs(OR=3.97,95%CI=1.38-11.5,P<0.01).ConclusionHigherMHRlevelsmaypredictMACEsdevelopmentafterelectivePCIinT2DMpatients.

  • 简介:Bioluminescenceimagingisakindofemergingdetectiontechnologyatcellular,molecularandgeneticlevel.Themostpopularbioluminescenceimagingmodelisdiffusionapproximation(DA).However,becauseoftheill-posednessoftheDA-basedinverseproblemandtheinstabilityofreconstructionalgorithms,thelocationaccuracyofthereconstructedsourcesislow.Radiativetransferequation(RTE),whichconsidersthedirectionofthephotonmigrationandtheeffectofabsorptionandscatteringintissues,canaccuratelyexpressthetransmissionofbioluminescentphotonsthroughthetissues.Inthispaper,westudiedthebioluminescenceimagingbasedontheRTE.2Dsimulationswereperformed,andquantitativeevaluationwasgivenbytheabsolutesourcepositionerror,therelativesourceareaerrorandtheminimumboundingbox.TheresultsoftheexperimentshowedthattheimagingqualitybasedonRTEwasbetterthanthatonebasedonDA.

  • 简介:Thismini-reviewpresentstheauthors'visiononthecurrentstatusandfuturetrendsinthedevelopmentofneuroprotectiveagentsworkingviaactivationofnuclearfactorerythroid2-relatedfactor2(Nrf2),andinparticular,viadisruptionofNrf2-Keaplinteraction.Therearetwoopposite'chemical'mechanismsunderlyingsuchactivation:thefirstoneisanon-specificcovalentmodificationofKeap1thiols,resultinginsideeffectsofvariedseverity,andthesecondoneistheshiftoftheNrf2-Kelch-likeECHassociatedprotein-1(Keap1)bindingequilibriuminthepresenceofacompetitiveandchemicallybenigndisplacementagent.Atthispoint,nodisplacementactivatorsexhibitsufficientbiologicalactivityincomparisonwithcommonNrf2activatorsworkingviaKeaplthiolmodification.Hence,thehopeintherapeuticsisnowlinkedtotheFDAapproveddimethylfumarate,whosederivative,monomethylfumarate,aswedemonstratedrecently,ismuchlesstoxicbutequallybiologicallypotentandanidealcandidateforclinicaltrialsrightnow.AnewlyemergingplayerisanuclearinhibitorofNrf2,BTBdomainandCNChomolog1(Bach1).ThecommerciallydevelopedBachlinhibitorsarecurrentlyunderinvestigationinourlaboratoryshowingpromisingresults.Inourviewpoint,theperfectfuturedrugwillpresentthecombinationofadisplacementactivatorandBachlinhibitortoinsuresafetyandefficiencyofNrf2activation.

  • 简介:Segmentingwholeheartfromcardiaccomputedtomography(CTimagescanprovideanimportantbasisfortheevaluationofcardiacfunctionandhelpimprovetheaccuracyofclinicaldiagnosis.Manualsegmentationisthemostaccuratemethodforcardiacsegmentation.Butitistimeconsumingandnotsufficientlyreproducible.However,cliniciansstillrelyonthismethodinpracticalapplications.Soafullyautomaticmethodisneededtoimprovethesegmentationefficiency.Thispapeproposesaregistration-basedautomaticapproachforthree-dimensional(3DsegmentationofcardiacCTimages.TheproposedmethodutilizesthesimilarityocardiacCTimagesbetweendifferentindividuals,andusesregistrationtoachievethesegmentation.Affinetransformationisfirstlyimplementedtoachieveglobalcoarseregistration.Then,cubicB-splinesareusedtorefinethelocaldetailsinlocallyaccurateregistration.Mutualinformation(Ml)isusedasthesimilaritymeasure,andadaptivestochasticgradientdescent(ASGD)astheoptimizationalgorithm.Oumethodisappliedtothedual-sourcecardiacCTimagestosegmentwholeheartExperimentalresultsshowthattheproposedmethodcanautomaticallysegmentwholeheartfromcardiacCTimages.

  • 简介:Inthispaper,weproposeamethodtoimproveanti-radiationcapabilitybycoatingheavymetalX-rayprotectionglasswithcompoundphotoniccrystallayers,basedontheuniquepropertyofphotoniccrystalthatlightcannotbepropagatedwithintherangeofbandgaps.Usingtheplanewaveexpansionmethod,wemadeatheoreticalstudyofparametersaffectingthebandgapstructuresofone-dimensionalphotoniccrystals.Basedonthefindings,wechoseappropriatematerialsandcompoundstructureofphotoniccrystalsoastogethighX-rayreflectioncoatingphotoniccrystallayers.Bythismethod,thereflectionratewithinX-raywavelengthcanreachthemaximumvalueof100%,andtheaveragevalueofover90%.Evenlow-costheavymetalX-rayprotectionglassofabsorptioncoefficientvaluecanachievethedesiredeffect.Thus,thismethodgreatlydecreasestheanti-radiationrequirementsofheavyX-rayprotectionglass.

  • 简介:Objective:ThisstudyaimstoestablishamethodforhighlyparallelmultiplexeddetectionofgeneticmutationsinChineselungcancersamplesthroughAgenaiPLEXchemistryandmatrix-assistedlaserdesorptionionizationtime-of-flightanalysisonMassARRAYmassspectrometryplatform.Methods:Wereviewedtherelatedliteratureanddataonlungcancertreatments.Wealsoidentified99mutationhotspotsin13targetgenescloselyrelatedtothepathogenesis,drugresistance,andmetastasisoflungcancer.Atotalof297primers,composedof99pairedforwardandreverseamplificationprimersand99matchedextensionprimers,weredesignedusingAssayDesignsoftware.Thedetectionmethodwasestablishedbyanalyzingeightcelllinesandsixlungcancerspecimens.TheproposedmethodwasthenvalidatedthroughcomparisonsbyusingaLungCarta~(TM)kit.ThesensitivityandspecificityoftheproposedmethodwereevaluatedbydirectlysequencingEGFRandKRASgenesin100lungcancercases.Results:Theproposedmethodwasabletodetectmultiplexgeneticmutationsinlungcancercelllines.Thisfindingwasconsistentwiththeobservationsonpreviouslyreportedmutations.Theproposedmethodcanalsodetectsuchmutationsinclinicallungcancerspecimens.ThisresultwasconsistentwiththeobservationswithLungCarta~(TM)kit.However,anFGFR2mutationwasdetectedonlythroughtheproposedmethod.Themeasuredsensitivityandspecificitywere100%and96.3%,respectively.Conclusions:TheproposedMassARRAYtechnology-basedmultiplexmethodcandetectgeneticmutationsinChineselungcancerpatients.Therefore,theproposedmethodcanbeappliedtodetectmutationsinothercancertissues.

  • 简介:WithagreatdifferenceintherapeuticeffectsofMahuang(MH,thestemsofEphedrasinica)andMahuanggen(MHG,therootsofEphedrasinica),chemicaldifferencesbetweenMHandMHGshouldbeinvestigated.Inthepresentstudy,gaschromatography-massspectrometry(GC-MS)-basedplantmetabolomicswasemployedtocomparevolatileoilprofilesofMHandMHG.TheantioxidantactivitiesofvolatileoilsfromMHandMHGwerealsocompared.32differentialchemicalmarkerswereidentifiedaccordingtothevariableimportanceintheprojection(VIP)valueoforthogonalpartialleastsquaresdiscriminantanalysis(OPLS-DA)andPvalueofMann-Whitneytest.Amongthem,chemicalmarkersoftetramethylpyrazine(TMP)andα-terpineolwerequantified.TheircontentsweremuchhigherinmostMHsamplescomparedwithMHG.TheantioxidantassaydemonstratedthatMHhadsignificantlyhigherfreeradical-scavengingactivitythanMHG.AlthoughMHandMHGderivedfromthesamemedicinalplant,therewasmuchdifferenceintheirvolatileoilprofiles.MHsampleshadsignificantlyhighercontentoftworeportedpharmacologicallyimportantchemicalmarkersofTMPandα-terpineol,whichmayaccountfortheirdifferentantioxidantactivities.

  • 简介:Objective:ToevaluatethefeasibilityofDNAimagecytometry(DNA-ICM)asaprimaryscreeningmethodforesophagealsquamouscellcancer(ESCC).Methods:Atotalof5,382localresidentsaged40–69yearsfromthreehigh-riskareasinChina(LinzhouinHenanprovince,FeichenginShandongprovinceandCixianinHebeiprovince)from2008to2011wererecruitedinthispopulation-basedscreeningstudy.And2,526subjectsdeclinedtoreceiveendoscopicbiopsyexaminationwithLugol'siodinestaining,while9and815subjectswereexcludedfromliquid-basedcytologyandDNA-ICMtestrespectivelyduetoslidequality.Finally,2,856,5,373and4,567subjectswereenrolledintheanalysisforendoscopicbiopsyexamination,liquid-basedcytologyandDNA-ICMtest,respectively.Sensitivity(SE),specificity(SP),negativepredictivevalues(NPV)andpositivepredictivevalues(PPV)aswellastheir95%confidenceintervals(95%CI)forDNA-ICM,liquid-basedcytologyandthecombinationofthetwomethodswerecalculated.Receiveroperatingcharacteristic(ROC)curveswereappliedtodeterminethecutoffpointofDNA-ICMforesophagealcancer.Results:DNA-ICMresultsweresignificantlycorrelativewithesophagealcancerandprecancerlesions(χ~2=18.016,P<0.001).Thecutoffpointswere5,802,5,803and8,002basedondissimilarpathologicaltypesoflowgradeintraepithelialneoplasia(LGIN),highgradeintraepithelialneoplasia(HGIN),andESCC,respectively,and5,803waschoseninthisstudyconsideringtheSEandSP.TheSE,SP,PPV,NPVofDNA-ICMtest(cutoffpoint5,803)combinedwithliquid-basedcytology[thresholdatypicalsquamouscellsofundeterminedsignificance(ASCUS)]wereseparately72.1%(95%CI:70.3%-73.9%),43.3%(95%CI:41.3%-45.3%),22.8%(95%CI:21.1%-24.5%)and87.0%(95%CI:85.7%-88.3%)forLGIN,85.7%(95%CI:84.3%-87.1%),41.3%(95%CI:39.3%-43.3%),4.6%(95%CI:3.8%-5.4%)and98.9%(95%CI:98.5%-99.3%)forHGIN,and96.0%(95%CI:95.2%-96.8%),40.8%(95%CI:38.8%-42.8%),1.7%(95%CI:1.2%-2.2%)and99.9%
